Our phone support offers, on-call problem solving, crisis prevention, safety planning, and emergency support.
A trained emergency response team member is always ready and able to respond in the client’s home or community, offering virtual support, in-person support, and daily check-ins on a case-by-case basis.

How We Help

Eligibility
An individual may be eligible to receive 24-Hour Emergency Support if they are a senior or individual with a disability enrolled in Medicaid or using private pay.
This service does not replace the use of 911 when necessary.
Eligibility Requirements
To qualify for Housing Stabilization Services, applicants must meet the following criteria:
✅18 years old or older
✅Must be on Medical Assistance (MA)
✅Must have a documented disability or disabling condition
✅Homelessness or at risk of being homeless
