  • Elderly Waiver (EW) Program

  • Alternative Care (AC) Program

  • Essential Community Supports (ECS) Program

If you or someone you know meet these criteria, or you're unsure about whether or not you qualify for services, please contact us.

To qualify for Housing Stabilization Services (HSS), applicants must meet the following criteria:

 

  • 18 years old or older

  • Must be on Medical Assistance (MA)

  • Must have a documented disability or disabling condition

  • Homelessness or at risk of being homeless

 

Referrals can come from the following providers:

  • Case Managers and Social Workers

  • Health Care providers qualified to confirm the disability or disabling condition

  • Mental Health Professionals

  • Disability Service Providers

  • Substance Use Disorder Providers

  • Homeless Outreach Workers / Housing Navigators

  • Self-referral: individuals who meet the eligibility criteria and are in need of HSS

  • We accept referrals from other housing and consultation agencies as well, please contact us.
