Housing Stabilization Services (HSS)
Housing Stabilization Services, launched July 20, 2020 in Minnesota.
Housing Stabilization Services are for people with disabilities, including mental illness and substance abuse disorder, who are:

Homeless or at risk of becoming homeless

Living in institutions or other segregated settings, or are at risk of living in those settings.
The purpose of these services is to:

Support an individual's transition into housing,
-
Increase long-term stability in housing in the community, and
-
Avoid future periods of homelessness or institutionalization.
Housing Consultation
Housing Consultation is a service to develop a person-centered plan for individuals without Medical Assistance (MA) case management services. These services may include:

Developing a housing focused person-centered plan based on the person’s needs, wants, and goals for housing

Help an individual make an informed choice in their housing transition or sustaining services provider

Offering resources related to non-housing goals

Coordinating with other service providers already working with the person

Housing Transition
Housing Transition is a service that supports an individual finding housing and moving into housing.
We help with:

Housing research, options, and application process

Develop a budget

Help find funding for deposits and moving expenses

Helping the individual organize their move

Understanding a lease
Housing Sustaining
Housing Sustaining is a service that helps individuals maintain housing.
We help with:

Education and training to help develop a positive relationship with landlords, property managers, and neighbors

Transportation
-
Assist with various housing benefits
-
Problem-solving to maintain housing stability

Eligibility Requirements
To qualify for Housing Stabilization Services, applicants must meet the following criteria:
✅18 years old or older
✅Must be on Medical Assistance (MA)
✅Must have a documented disability or disabling condition
✅Homelessness or at risk of being homeless
